I like the structure you create with the window and curtains in the middle. Also a great way to bring the elements of the people, pills, and screens into the composition. I wonder a bit about the opacity levels of all the elements in the window--anything to be had by emphasizing some or other elements? I also wonder about the coin/medal. It feels a bit floaty given how well everything else is woven into the image.
